Green Rx Produce Prescription Program

Patients of Mercy Health who have screened positive for food insecurity, and have been diagnosed with a chronic health condition, are referred to the Green Rx program upon a visit to their primary care physician. After being screened positive for food insecurity, patients and families will be enrolled for food assistance through their primary care provider and receive a voucher for food, redeemable at the West Ohio Foodbank. Patients will receive a box of shelf stable food at their primary care physicians office, and then visit the foodbank to receive access to fresh produce. After 8 weeks of healthy food distribution, the patient will return to their physician for a vital sign check-up, to see if the food access has improved the quality of health for the patient.
